Hospital Discharge Planning Project (2008)

The purpose of this project was to increase choice in care settings at time of discharge for older adults and adults with disabilities. The goal was to increase understanding of the discharge process; identify choice in care settings after discharge; and identify strategies to increase choice. Outcomes included a description of the hospital discharge planning process and recommendations for systems change.
